That rope grab on the 18th floor had me questioning my whole morning

I clipped my lanyard to what I thought was a solid rail tie on a hydro in Minneapolis, and the whole bracket just spun loose, about 3 seconds before I put weight on it. Guess I learned to always spin-check the anchor nut before trusting it, anyone else had a close call with pre-torqued hardware that wasn't?

You ever heard of a guy testing a ladder jack on a job site in St. Paul, and the whole clamp assembly just let go while he was setting his plank? He said it sounded like a gunshot when the spring shot out, and he nearly went over with it. Turned out someone had swapped in a used bolt that looked fine but was stripped clean, and it was only holding by about two threads. He checks every piece of hardware with a wrench now, even the stuff that comes straight out of the box.
