Sling angles get ignored until something drops

Watched a crew in Houston rig a 12-ton load with 45-degree slings last Thursday, guy said "it's fine, we've done it before." That math puts way more stress on the picks than they planned, and I've seen a shackle pin bend from it. Has anyone else had to stop a job to re-rig because someone skipped the angle check?
