That splice box in Bellevue taught me to stop trusting the diagrams

Last Thursday I opened up a 5 year old aerial tap and the labels inside were completely backwards from the actual wiring. I spent 40 minutes tracing a dead drop that was burning up a customer's modem because I followed the print, not the wire. The previous installer had swapped the center conductor and braid at the connector, the drawing showed it clean. I cut everything and reterminated it by hand, fixed the signal in 10 minutes after that. How do you guys handle outdated prints on older plant, do you trust the tag or the tone every time?

Wait, do you check the tags on every splice you open up, or just when somethings already acting up?
