I keep seeing people push for a 'flat' structure in small teams and it's a bad idea

We tried running our 12 person marketing group with no clear leads for about 8 months, and project deadlines started slipping by weeks. The problem wasn't people being lazy, it was that nobody had the clear job to make a final call when ideas clashed. Has anyone actually made a flat setup work without everything taking three times as long to finish?

Our flat team works great because we trust each other to make quick calls.

That's the only way it can work honestly. Without trust, everything just gets stuck waiting for approval. You've got the right idea.
