Had a 3-inch steam line let go at 150 psi during a shutdown in Toledo last Tuesday.

We were doing a routine isolation for a valve swap. Pressure was supposed to be zero, but a bypass valve on a different loop was left cracked. Heard that hiss, then a bang. The whip took out a handrail and a light fixture. Nobody was in the path. The noise alone was enough to make your heart stop. It was a paperwork screw-up, pure and simple. The lockout tagout sheet didn't list that particular bypass. Always double-check the entire system, not just the tags in front of you. What's the most thorough way you guys verify zero energy before cutting in?

Used to just trust the tags, but after a close call like that, physically tracing it is the only way.
